Freedom of Information Requests

he Freedom of Information Act (FOI) gives you the right to ask for information held by a public authority. Anyone can ask for this information, and we’ll provide it if it is not exempt from disclosure.

Data protection law also gives you the right to ask for information which we hold about yourself. If you are requesting medical information on yourself or someone you have authority to represent, then this is called a ‘Patient Record Request’, and can best be accessed via the NHS app, as described here

For FOI requests not relating to your medical information, or for information not in your medical record, please contact us via the contact page. Alternatively you can send a letter to any surgery address, or email us at hnyicb-ny.haxbypatientexperience@nhs.net. We cannot accept FOI requests by phone.
