Our Non-NHS services

At Haxby Group, there are some services we can provide that are not funded by our NHS contracts. For most of these services, there will be a fee.

Patient record requests

The best way to have ongoing access to your patient record is to sign up for the NHS App or login via the NHS website. These can provide access to most of your patient record information, including your GP Health Record. For more details of how to do this, please see the NHS guidance.

If you require further information, a complete copy of your records (subject access request) can be requested from Haxby Group and will be provided electronically at no charge, in line with General Data Protection Regulation (GDPR). For hard copies, there may be a fee.

Medical reports

Haxby Group can provide various types of report on request – for example, life and travel insurance; DVLA; benefits; housing; and occupational. We can only provide reports within the area of expertise of our GPs. We are unable to give you more specialised information on your health.

In-person medical assessments

For some reports, you will need a face-to-face medical assessment. As this takes longer than completing a paper report, the fee is higher. Examples include HGV licensing; taxi licensing; and adoption medicals. These assessments can only be carried out in specified appointment slots. They cannot form part of a routine GP appointment.
