Register as a patient with Haxby Group
To receive the outstanding care that we give to our patients, please register with us. We can register patients who live within our practice boundaries in York.
Anyone in England can register with a GP surgery. You do not need proof of address or immigration status, ID or an NHS number.
All registration applications can be completed online, or at your nearest Haxby Group surgery.
When you register, it’s helpful to use your NHS number, if you have one. You can use the NHS website to find your NHS number.
